INSURANCE INFORMATION:

I no longer accept HMO and health insurance plans.

IF YOU HAVE A PPO PLAN (Preferred Provider Option— you can choose who to see and the plan reimburses a percentage): I receive your payment at the time of our meeting. I'm happy to provide you with a Superbill containing the codes and receipts you will forward to your plan to get the reimbursement rolling. Your insurance company will provide you the details of how much reimbursement you might receive. I recommend checking in with them so you will know what to expect, and whether or not paying me up front for a percentage of reimbursement is within your financial reach. In our initial contact, please let me know if you will need receipts or codes to claim reimbursement.

ACCEPTABLE FORMS OF PAYMENT:

CREDIT CARD to be stored in secure service taken during first session and billed during the session week.

  • Also acceptable: Zelle, check, cash, Venmo (@judY-silvan), Paypal (judysilvan@mac.com/Friends and Family option-or client is responsible for fee)‘Transfer’, Zelle and other direct-transfer applications. payment to be received at the time of the services. If you choose to use your credit card I will bill for each session and this will occur during the week of the session (before the weekend following). Typically it is same day billing, either the morning the session will occur, or just after the work day ends.

*Patient is responsible for any fees taken out of the payment by PayPal; you can click “friends and family” to avoid the fee.

* If your care has been inactive for one to three months, or there is no agreed-upon plan to account for longer periods of gaps in care, it is assumed you are no longer in need of services. Your file will be closed and the therapist-patient relationship will terminate. Other grounds for termination include missed appointments, casual cancellations, abusive or violent behaviors, and failure to maintain financial responsibility.


* For new referrals who identify as Black, Indigenous or Transgender, I am able to slide my fee. Please feel free to let me know at the time of initial contact or in our initial appointment. Together we can decide on a fee that makes sense for you as gesture of reparations for exclusions and oppression in the field and beyond.

**Unfortunately I don’t offer any other reduced fee slots now other than for AEDP Research Patients; they come up on rare occasion. Please feel free to inquire.

Fees:

MY FULL FEE IS $350/50-55 minute session and $450/55-60 minute coaching session
*IF you are a single-adult earner or an AEDP therapist and cannot afford my full fee, a SLIDING SCALE is available to you. The slide goes from my full fee down a bit. Please ask if the need exists and you fit into either category.

50-55 minutes — First 3 sessions are an Assessment Period to decide if I am the best match for your needs going forward, or if a different referral is more aligned for you.

INDIVIDUAL THERAPY:
(50-55 minutes) $350
1.25 sessions (75 minutes) - $405
1.5 sessions (90 minutes) - $450

COUPLES THERAPY:

50-55 minutes - $350; no sliding scale for Couple Therapy; use above pro-rates for longer sessions.

AEDP SUPERVISION:
50-55 minute sessions with Sliding Fee Scale: $350-$285. (Please choose the amount you’re able to pay).

COACHING:

$250-30 minutes, 50-55 minutes remote Coaching Sessions - $450

COLLATERAL WORK:

$300/hour for collateral email, out-of-session paper work, including court appearances (billed in 15 minute increments)

CANCELLATION POLICY:
ONCE AN APPOINTMENT IS SCHEDULED: Patient, client or supervisee is responsible for payment.
EXCEPTION: DURING THE SAME BUSINESS WEEK, if both parties agree on a new time during the same week as original appointment, payment can cover the latter session and the initial session’s fee is waived.

If less than 48 hours notice is given, appointment can be cancelled and re-scheduled, and client is responsible for both hours’ fees.

Please email me to request an initial appointment, sharing a bit about what you are seeking in psychotherapy, coaching, or supervision. Please limit use of emails during course of psychotherapy to logistics.

PAYMENT IS DUE AT THE TIME OF APPOINTMENT.
**I require 48 business hours notice for changes and cancellations to waive fee for initial session.

For other session, cancelled session payment is waived— ONLY IF —another appointment is made during the SAME BUSINESS WEEK.
